Job Description:
A company is looking for a Care Manager (RN) to manage a
caseload of moderate to high-risk members.
Key Responsibilities
Evaluate member needs and recommend care plans for optimal
outcomes
Develop ongoing care plans and collaborate with providers to
address unmet needs
Provide education and resources to members and their families
regarding healthcare options and benefits
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure Compact State Licensure
required
Residents of Ohio (OH) or Delaware (DE) only
